Fines, sanctions and accidents
The following is a list of fines and sanctions (orders) imposed on StatoilHydro during 2008 and accidents that occurred during the group's operations in 2008. An order is a preventive policy instrument which has a binding legal effect on the recipient.
Fines:
- StatoilHydro was fined NOK 2 million in March 2008 for an accident that occurred on 26 April 2005 on Oseberg B, in which a drilling worker was seriously injured.
Orders:
- On 19 February 2008, the Petroleum Safety Authority Norway issued an order to StatoilHydro on the basis of its findings from an audit of load-bearing structures at Veslefrikk A and B carried out during the period 22 August to 6 November 2007.
- On 4 April 2008, the Petroleum Safety Authority Norway issued an order to StatoilHydro in connection with an incident that took place on 12 December 2007 in which a ruptured loading hose on Statfjord A led to 4,400 cubic metres of crude oil being pumped into the sea.
- The Petroleum Safety Authority Norway conducted an audit during the period 13 May 2008 to 30 May 2008 of StatoilHydro's management of health, safety and environment (HSE) and the use of integrated operations (IO) in drilling and well activities on the Heidrun TLP facility. Based on its findings during this audit, the Petroleum Safety Authority Norway issued an order to StatoilHydro on 5 August 2008.
- On 16 October 2008, the Petroleum Safety Authority Norway issued an order to StatoilHydro following its investigation of an incident which took place on 24 May 2008 in which a hydrocarbon leak occurred in one of the shafts of the Statfjord A facility in the North Sea.
- The Petroleum Safety Authority Norway issued an order to StatoilHydro on 17 October 2008 in connection with StatoilHydro's failure to provide documentation for contractor personnel at Mongstad.
Accidents:
- We had two fatal accidents in 2008. During a canoeing trip on a teambuilding exercise, one person drowned as he was snagged by the branch of a tree lying in a river. The second fatality occurred when casting off from the production platform South Pars No 9 in the Persian Gulf. A mooring line broke and struck a crew member on board the vessel Interservice.
- Two other serious incidents were air intrusion in a cracker at Mongstad and an oil leak in the Statfjord A shaft, both of which had the potential to develop into a major accident, since there was a risk of explosion.
- Large leakages of gas or other dangerous liquids always carry a major risk, and in 2008 the number of red incidents increased compared with the previous year. Dropped objects remain the single largest contributor to red incidents, and several actions have been taken to improve this, both at corporate and business area level.
